What is medication management?

Medication management is patient-focused care that encourages adherence to medication routine through medicine packaging according to the time doses need to be administered. Organising doses in small, individual packs such as sachets or blister packs helps carers and patients remember to administer medications. Dose Administration Aids assist in visually tracking medication uptake as the patient can clearly see when doses must be taken or if they have been missed. Medication management also involves any other support that may be needed to assist the patient’s optimal uptake of medication such as reminder notifications.

Benefits of medication management

  • Better health outcomes for patients especially seniors
  • Reduced chances of missed doses and/or incorrect doses
  • Decreased risk of health complications arising from administering doses at the wrong time
  • Making managing health of a loved one stress-free
  • Improved adherence with simplified packaging

Top tips for managing medication while travelling

Managing medication while travelling requires more attention and planning than just making sure you have packed enough for the trip. One of the most important factors to consider are time zones you will be travelling through and plan your doses accordingly. Weather conditions can affect the composition of medications and it is worth finding out beforehand if you need to make special arrangements for storage.

Before travelling, consult with your doctor any diets that may have an unfavourable impact on your regular medications. While travelling to new places, you may not always have control over what is available to eat. If you are required to take medication with meals, speak to your doctor about snack options to pack for the trip.

Here are some tips to simplify medication management at home

Understand suitability to diet

Some medications need to be taken with certain foods or must be avoided with particular food groups. It is best practice to understand how your diet can affect the effectiveness of your medications.
