What is a Home Medication Review

A Home Medicines Review (HMR) is a clinical process that considers the Patient’s medicines and health in order to enhance the Quality Use of Medicines (QUM) and reduce the number of adverse medicines events.

If you take five or more medications or have recently been in hospital, book a Home Medication Review with your Pharmacist.

What will happen at my HMR?

Your Pharmacist will:

  • talk through each of your medications
  • check how you take them
  • check all expiry dates
  • show you how to properly store your medication
  • give you tips to remember when to take them each and answer any questions you may have.

What do I need to do before my HMR?

Gather all your medications, including:

  • prescriptions
  • over-the-counter medicine
  • vitamins, supplements and herbal products

Remember to gather all forms, including:

  • oral
  • creams/ointments
  • inhalers
  • injectables
  • patches
  • ear and eye drops

What happens after an HMR?

Your Pharmacist will prepare a report and share it with your GP, who will decide if your treatment plan needs to be changed at all. It may be necessary to arrange a follow-up appointment with your GP to discuss the report prepared by your Pharmacist.

Should I book an HMR?

If you or someone you care for is taking more than five medications, has recently been in hospital, is feeling overwhelmed about managing medications, an HMR can assist. Mixing medications incorrectly can have undesired health consequences, so it’s a good idea to be sure of how to use multiple medicines.
